APOTEX – CLASS II RECALL – 09/09/09


DUE TO THE NUMBER OF RECALLS ADDRESSED IN THIS ALERT, INFORMATION
IS OUTLINED IN THE ATTACHED DOCUMENT.

Apotex Pharmaceuticals is initiating a voluntary CLASS II recall that addresses a non-
safety issue with the product due to fact that affected batches may not meet GMP
specs, and as a precautionary measure, is asking all retail pharmacies to now remove
the below listed LOT #’s for the 143 products on the attached list. Included are
controlled substances (Recalls 35-40 and 123-127 outlined in red).
Pharmacies are advised to quarantine and discontinue distribution of these lots. Further
instructions will come directly from your distributor.
RelayHealth has responded by implementing a NotifyRx™ safety alert informing
pharmacies of this recall at point-of-dispense when the claim is processed through the
Intelligent Network.
Beginning today, all pharmacies that participate in the NotifyRx messaging program will
see messages as detailed on the following attachment for the listed drugs.
